Congratulations on your acceptance into the UC Merced Early Childhood Education Center!
The following forms must be submitted prior to your child's first day at school during your orientation appointment. Please complete prior to orientation. If you are in need of paper forms versus downloading these from our website, please contact us.
In addition to these forms, immunization records must be brought to the orientation. During your orientation, we review all paperwork to ensure it is complete, introduce policies, answer any questions and allow you time to meet your child’s teacher. Please allow between 1-2 hours for your orientation.
If you have any questions regarding any of the forms, please feel free to contact the ECEC.
Please do not fill these forms out until you receive notification from the ECEC staff that you have been accepted into the program.
